I never really found the missing files but they weren't important files anyway.
The problem did go away after installing the latest CS update from Digi. I never seemed to have the missing files or fades message again. At the time Digi never acknowledged the problem even though they posted a fix, I don't know if they do now. I had this today with one of the pro tools training exercises. I think it was because the session got stored on a novell server - totally screwed it up.
Have you tried this? I know you've said you've done all of that individually Jay, but did you combine trashing prefs with starting a new session?
I hope you get it solved, Drews suggestion seems the last viable option to me. Some other things that you could try, saving to an older type of PT session though you probably already tried that or try to export as an AAF and then import it to a new session. But I have a feeling those things won't work, because you already said in the beginning that Pro Tools does not show any blue empty regions, meaning PT 'has forgotten' there should be audio on the track and any save in between would have saved that state.
I haven't heard of anything like this before and a lot of the responses in this thread seem to come from people who aren't super familiar with PT issues or didn't read your post. Hi I think I might have a similar problem, only the files show up missing "normally", with the standard file missing window and blue files in the session.
I can find the files in the Audio Files folder, but the file is somehow corrupted and will not play, import, relink, neither on the studio system or on alternative systems. I can tell by the file size that the sound has been recorded though, and it also takes time to copy and upload the file to other disks.

This is because Pro Tools stores all the region information within the Session and not the parent audio file. Jay M. I have heard of Nuendo users with corrupt project files who sent it to Steinberg and they were able to fix it. Maybe digidesign will do the same. The friend of mine who had that happen learned a valuable lession!

After a forced power down and reboot, PT worked fine as did the Imac, but The error message states something to the effect I'm home now, not in the studio "Pro Tools can't open a session done on a previous version".
